Re: New beneficiary designation process

As previously announced in UC HR/Benefits Review and UCSC Benefits Update, UC is pleased to announce the availability of a new online process for naming retirement and insurance plan beneficiaries. Detailed information regarding the new online process can be found at http://www2.ucsc.edu/staff_hr/benefits/update040401.pdf

We would like to remind you that for reasons of time and budget, UC entered only primary UCRS beneficiary information of name, date of birth, and relationship; no data was entered for life and AD&D insurance. We encourage you to review your retirement beneficiary designations for accuracy and designate your insurance beneficiaries. To take advantage of this new process, select "Your Benefits Online" on the right side of the At Your Service home page (http://atyourservice.ucop.edu/). After you log on, you’ll find a link—"View Your Beneficiaries"—under both "Your Money" and "Your Health & Welfare." A new paper form has been developed for those who can’t use the online application. Forms are available from the campus Benefits Office (831-459-2013) or UC Customer Service Center (1-800-888-8267).

By 2006, UC plans to obsolete any beneficiary designations not submitted using the new online application or new forms and to base beneficiary payments on order of succession unless designations have been identified through the new process.
